Translation Challenges Aurimar

1.The source text references and the documentation problem: one of the issues regarding the translation process was the documentation problem. This procedure went beyond reviewing references, and involved searching for equivalents for certain phrases or terms appearing in the text, which refer to specific definitions of the subject area. In order to do so, I had to investigate:

a)differences between biceps and rotator cuff

b)the meaning of an allogenic transplant

c)the reference of WST-1 proliferation assay

2.Specialized terminology: the use of specialized terminology led me to consult some expressions, phrases and words, such as:

a)mesenchymal stem cells

b)reverse shoulder prosthesis

3.Use of passive voice: the source text uses passive voice to offer information regarding references. In this sense, the solution put in place in the majority of the cases was the reflexive passive voice to ensure the logical arrangement of the grammatical elements. Some examples of this use are the following expressions:

a)“Muscle-tendon-transfer operations are not only limited” translated as “Las intervenciones de transferencia músculo-tendón no solo son limitadas”.

b)“Until now, however, it has not completely been understood which tendon tissue is to be used to isolate autologous TDC” translated as “Aún así, hasta ahora no se ha entendido totalmente qué tejido tendinoso se utilizará para aislar las CDT autólogas”.

I faced several challenges while I was translating this medical text. All related to the use of specialized terminology:


  1. Tears: In order to understand the term, I searched in the free dictionary by farlex, the meaning of the word tears: “A rent or disruption of a flattened tissue or surface”. As well, I had to consult the meaning of several terms, in the same field: cuff, allogenic, mesenchymal, osteogenic, strain, cell culture, chondrocytes, scaffold and decorin.

Unknown Terminology: Being the medical language a specialty language and coming from the area of Statistics and Social Sciences, there is whole disciplinary terminology that I ignore and it demands performing of my documentary competence, which acts as a transversal axis on the technical translation process. It is present in terms such as: anterior cruciate ligament, cell cultures, cell passaging, collagen matrix scaffold, decorin, donor site morbidity, massive rotator cuff tears, mesenchymal stem cells, reverse shoulder prosthesis.

  1. Choosing the right one between multiple synonyms for the Spanish translation of “tear”:  desgarro, desgarre, desgarramiento, ruptura, rotura. This hesitation between the available nouns and whose of them would result acceptable inside medical terminology reveals some lack of communicative competence in two languages, according to the PACTE Holistic Model, that must be solved as soon as possible. As a general remark, I must say that in translating this hypothetical case, I made an intensive use of the communicative competence in order not to miss out on meaninglessness, nonsense, etc. in the description of the culture procedure and the passage of mesenchymal stem cells -and of another origin- in the treatment of tears of the rotator cuff of the shoulder, consulting documentation on the subject and carrying out searches and comparisons of content and meaning that I considered pertinent. Solution: Use of the terms “desgarro” and “desgarre”, as the most common used in medical journals, so I used it indistinctly in the translation.



  1. Finally, I cannot ignore the participation in this assignment, of interpersonal competence (this already, within the model of D. Kelly, or included in the strategic competence of the PACTE group model) and I must acknowledge the selfless dissemination of videos, pictures and documentation on the topic of treatments for rotator cuff ruptures by our teammate Thelma Rodríguez, always generous and attentive. Her references helped to clarify several of our doubts in the performing of the translation. During the translation process, the most important challenge encountered was: Specialized terminology such as Tears, Cuff, Mesenchymal, tenocytes, tensile strain, tendinous tissue, musculoskeletal, osteoblasts, chondrocytes, decorin. Estos desgarros, siempre que sea posible, deben tratarse mediante reconstrucción quirúrgica. Sin embargo, el manejo terapéutico de estos desgarros es difícil ya que la satisfacción postoperatoria del paciente es impredecible y oscila entre el 27% y el 74% [9]. Otro problema es la alta tasa de re-rupturas del manguito, que se pueden ver en estudios radiológicos en aproximadamente el 60% de los casos después de la cirugía [10]. Por lo tanto, se necesitan nuevas estrategias para mejorar el resultado en la calidad de vida de los pacientes con desgarros masivos del manguito rotador. Una de estas estrategias son los trasplantes alogénicos de tendones, los cuales no se utilizan de forma rutinaria en la práctica clínica actual y siempre conllevan el riesgo de infecciones y rechazos [11]. Las operaciones de transferencia de músculos y tendones no solo son limitadas, sino que también muestran resultados insatisfactorios [12]. El único tratamiento definitivo en la actualidad es el reemplazo articular artificial con una prótesis reversa de hombro. En este caso, el aflojamiento de la prótesis puede causar graves problemas con el tiempo, situación que se observa en un número notable de casos [13,14].

Todos estos intentos parecen tener complicaciones que limitan su uso clínico y, por tanto, se necesitan nuevas soluciones. Un nuevo enfoque innovador para tratar desgarros masivos del manguito rotador en el futuro podría ser la ingeniería de tejidos tendinosos (ITT). Este enfoque tiene como objetivo crear tejido tendinoso de buena calidad, mediante la siembra de células en andamios in vitro, con el objetivo de asumir la función específica del tejido afectado tras la implantación in vivo. Las células madre mesenquimales y los tenocitos se utilizan con frecuencia como fuente celular en la ITT; sin embargo, las células mesenquimales parecen estar limitadas en la medida que es posible la diferenciación en múltiples tipos celulares [15]. También hay evidencia de que las células madre mesenquimales responden con orientación osteogénica si se les aplica tensión de tracción cíclica [16]. Por lo tanto, las células derivadas de tendones (CDT) parecen ser el tipo de célula más adecuado. Hasta ahora, sin embargo, no se ha comprendido completamente qué tejido tendinoso se debe utilizar para aislar CDT autóloga. Se deben cumplir los siguientes criterios para obtener resultados óptimos: en primer lugar, debe haber una cantidad suficiente de células disponibles después del procedimiento de aislamiento celular; en segundo lugar, las células deben conservar un fenotipo estable durante el pase de células; en tercer lugar, las células deberían crecer notablemente en los cultivos celulares; y finalmente, la pérdida de tejido tendinoso como fuente de células para la CDT solo debería causar poca morbilidad en el sitio donante [17].  

El objetivo de este estudio fue comparar las células derivadas de tres tejidos tendinosos y ligamentosos nativos diferentes: el tendón de la cabeza larga del bíceps (TCLB), el tendón del músculo semitendinoso (TMS) y el ligamento cruzado anterior (LCA). Se realizó un análisis de expresión génica en importantes proteínas estructurales de la matriz extracelular (MEC) de los tendones y se compararon los resultados con células de otro origen músculo- esquelético como osteoblastos, condrocitos y fibroblastos. La expresión de las principales proteínas estructurales, el colágeno tipo I y la decorina, se investigó más a fondo mediante la reacción en cadena polimerasa en tiempo real (PCR). Los resultados sobre el TCLB fueron publicados previamente por nuestro grupo de estudio [18,19]. La proliferación de CDT de TCLB fue analizada en un nuevo andamio de matriz de colágeno (Parche de Hombro, TETEC, Reutlingen, Alemania) utilizando el ensayo de proliferación WST-1”.
